Office and Prescription Refill Hours
Our offices are open between 8 a.m. - 5 p.m. Monday-Friday. Physician schedules may vary. A 24-hour service will answer your calls when the office is closed. Emergency calls will be forwarded to the on-call physician.

Requests for prescription refills will only be handled during the following office hours:

  • Monday – Thursday, between 9 a.m. and 5 p.m.
  • Friday, between 9 a.m. and noon
  • Prescriptions cannot be refilled after hours, on weekends or on holidays.

Appointments
Patients will be seen by appointment only, except in case of an emergency. Please call as far in advance as possible to schedule an appointment. If you cannot keep your appointment, please notify the office at least 24 hours in advance of your appointment time. There is a $25.00 charge if you fail to show for a scheduled appointment or cancel with less than 24-hours’ notice.

All patients are responsible for co-payment at time of service. We reserve the right to reschedule your appointment if payment is not made at the time of service. Patients that present as self-pay will receive a discount on specified services when services are paid in full on the day of visit.

For new patients, please bring your insurance card with you so that we may obtain a copy. If applicable, also bring all current X-rays to your appointment. We may not be able to care for you if we do not have all of the information available at the time of your appointment.

Patient Registration
New Patients: If you are a new patient, thank you for choosing AZ Center for Bone & Joint Disorders for your health care needs. Prior to your first visit to your office, please contact our patient registrar at (602) 553-3113 (Central Phoenix) or (480) 354-5900 (Mesa) to register as a new patient. Please be prepared to provide insurance and demographic information during the registration process. In addition, please see the “General Forms” section below. Returning Patients: please notify the receptionist of any change in name, address, telephone number or insurance coverage prior to your appointment.

Insurance Information
At AZ Center for Bone & Joint Disorders, we accept most major insurance plans. Please visit our Insurance Plans web page for more information. Please ask about our out-of-network and cash pay options.

If you participate with a Managed Care insurance plan, PPO or AHCCCS, you may be required to contact your primary care physician for a referral to our office.

General Forms
The following are links to various forms that require completion prior to your visit and/or surgery. To expedite the registration process, please download the appropriate form as directed by your physician and/or AZCBJD staff member and bring the completed form(s) to your scheduled visit.


Disability Forms
At AZ Center for Bone & Joint Disorders, we are happy to help you complete disability forms for a minimal fee. Please understand these forms cannot be completed while you wait. Upon request, we will complete disability forms as quickly as possible and mail the forms your home or your insurance company.

Release of Medical Information
As per the Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy rule, your medical records are kept strictly confidential. Before we can release any records, you must first sign an authorization form.

Please note, you will be charged a fee for the preparation of special reports and for the copying of medical records and X-rays for attorneys and insurance companies. Our office must have at least a 24-hour notice to prepare copies of medical records and/or X-rays. If you are scheduled for a diagnostic test, please request X-rays before leaving the office.

Diagnostic Testing
If you are scheduled for a diagnostic test (blood test, CT scan, nuclear medicine scan or arthrogram), you must take copies of your X-rays with you to the testing facility. When appropriate, please request copies of your X-rays before leaving the office.

Following any diagnostic testing, please phone our office at (602) 553-3113 (Central Phoenix) or (480) 354-5900 (Mesa) to make a follow-up appointment to discuss the test results. Please make arrangements with the test facility to pick up the written report of your results, as well as the actual films. Bring the results and the films to your follow-up appointment.

X-Rays
If you are considering having surgery at the AZ Center for Bone & Joint Disorders, we respectfully request that you have X-rays taken prior to your surgery. Preoperative X-rays are essential as they enable our surgical team to appropriately plan the best surgical approach and assess the need for additional bone grafts or other materials.

If you choose to have X-rays completed at a different facility, please plan to provide our office with a set that we may keep for our records. Please note, only X-rays with diagnostic quality will be accepted. If the X-rays are of inadequate quality, you must allow us to take X-rays at our office. Also, if you do not have X-rays at the time of your appointment, you will be asked to have X-rays taken upon your arrival at the office. Following surgery, X-rays will be taken and kept in our office.
